A representative from the Sangamon County Circuit Clerk's passport office will help guide you through the process of obtaining a passport.
You will need:
1. Certified copy of your birth certificate
(no photocopies or hospital certificate)
2. Valid photo ID
3. Approved passport photo
(photos can be taken the day of the event for $10.00, 2x2 inches)
4. Certified copies of court orders for a name change, custody or adoption decrees
5. Two checks or money order to pay applicable fees
